【完成本申請表後即附修業證明書及成績單一份;第6 週前完成申請者退費2/3;第7 週始退1/3;第13 週始不退費】

3.學位證明書Re-issuance of Chinese or English Diploma

(1)1樓中庭「自動化服務系統」繳費,即可產生申請表及繳費收據→持申請表及身分證影本繳至註冊組,約5個工作天。

Pay at the "Automated Service System" in the atrium on the 1st floor, application form and payment receipt will be generated Submit the application form and a copy of your ID card to the Registration Section, it takes about 5 working days

(2)填寫「學位證明書」申請表 持申請表至出納組繳費 →持申請表及身份證影本繳至註冊組,約5個工作天。

Fill out the "Degree Certificate" application form Bring the application form to the cashier team to pay the fee Submit the application form and a copy of your ID card to the Registration Section, it takes about 5 working days

4.更改學籍資料Change student status information

填寫「更改學籍基本資料申請表」並檢附戶籍謄本或國民身份證影本(請攜帶正本驗後退回)

Fill out the "Application Form for Change of Basic Information on Student Status" and attach a copy of the household registration or a copy of the national ID card (Please bring the original copy for inspection and return)

5.補發學生證Reissue student ID card

(1)1樓中庭「自動化服務系統」繳費,即可產生申請表及繳費收據 →持申請表至註冊組,約5個工作天。

(2)填寫「補發學生證申請表」→持申請表至出納組繳費→持申請表至註冊組,約5個工作天。

Fill out the "Reissue of Student ID Card" application form Bring the application form to the cashier team to pay the fee Submit the application form to the Registration Section, it takes about 5 working days

6.中文成績單Apply for Chinese transcript

(1)1樓中庭「自動化服務系統」繳費,即可現場印製成績單。

Pay at the "Automated Service System" in the atrium on the 1st floor, transcripts can be printed on-site

(2)填寫「中文成績單申請表」→持申請表至出納組繳費 繳至註冊組,約1個工作天。

Fill out the "Chinese Transcript Application Form", Bring the application form to the cashier team to pay the fee Submit the application form to the Registration Section, it takes about 1 working days

7.英文成績單Apply for chinese transcript English transcript

(1)1樓中庭「自動化服務系統」繳費,憑收據至註冊組領取英文成績單。

Pay at the "Automated Service System" in the atrium on the 1st floor, present the receipt to the registration section to collect your English transcript

(2)填寫「英文成績單申請表」→持申請表至出納組繳費 持申請表及護照影本(核對英文名字用) 繳至註冊組,約5個工作天。

Fill out the " English Transcript Application Form", Bring the application form to the cashier team to pay the fee Submit the application form and copy of passport (for verification of English name) to the Registration Section, it takes about 1 working days

8.復學:

【於通知期限內辦理,特殊情況得延至開學前辦理】

Process within the notice period, In special circumstances, the application may be postponed until before the start of school.

9.畢業生離校

於規定辦理時間內,登入「校務資訊系統-離校審核查詢」,且依各單位規定上傳應繳文件。

Within the prescribed processing time, log in to the "School Information System-School Leaving Review Inquiry", and upload the payable documents in accordance with the regulations of each unit

【修習科目成績全部送達並符合畢業資格後方可辦理離校手續並領取紙本學位證書及數位學位證書。】

All the results of the subjects studied have been sent and meet the graduation qualifications then can you go through the formalities for leaving school and receive paper degree certificates and digital degree certificates.

10.保留入學資格Maintain admission

填寫「保留入學資格申請表」並檢附表件內所需文件 ,新生於註冊截止日前完成申請手續。

Fill out the "Application Form for Retention of Admission Eligibility" and check the required documents in the attachment. Freshmen must complete the application procedures before the registration deadline.

新生有下列情形之一者,得於繳費註冊截止日()前,向教務處申請保留入學資格。

New students who meet any of the following circumstances may apply to the Academic Affairs Office to retain their admission qualifications before the registration deadline (inclusive)

一、依兵役法規定服役而不能按時入學並檢具相關證明文件者。

Serve in accordance with the provisions of the Military Service Law and are unable to enroll in school on time and obtain relevant certification documents

二、因病、特殊事故不能按時入學並檢具相關證明文件者。

Unable to enroll on time due to illness or special accidents and have to submit relevant certification documents.

三、因懷孕、分娩或撫育三歲以下子女並持有證明者。

Due to pregnancy, childbirth or raising a child under three years old and holding a certificate

四、因突遭重大災害者,經教育主管機關認定屬影響正常學習者。

Suffer sudden major disasters are deemed by the education authorities to have affected normal learners.

11.科目學分抵免Subject credit transfer

登錄本校校務資訊系統,點選「抵免申請作業」,詳閱「國立金門大學學生辦理科目學分抵免重要事項」說明,依需求點選「新增抵免/抵修/免修資料」,選擇欲抵免/抵修/免修科目後,點選「輸入完成,存檔」,列印申請表並檢附原就讀學校歷年成績單及適用年度之課程規畫表→ 至各開課單位核章 →送繳系所辦公室核章 →繳交至註冊組 (請依行事曆公告日期辦理為準)

Log in to school information system, click "Credit transfer Application", Please read the "Important Matters Regarding Course Credit Transfer for National Kinmen University Students" in detail, click "Add credit transfer subject Information" according to your needs, after selecting the subjects you want to be transfered/deducted/exempted from, click "Input completed, and save",print the application form and attach the transcripts from the original school and the course planning sheet for the applicable year. To each course unit for approval To the department office for stamp verification Submit to the registration section (Please refer to the calendar announcement date for processing)
